If you need to book a review or assessment meeting (either in person or remotely) this needs to be done by emailing our Assessments and Contracts Team with the email title “scheduled assessment/review request” and this is to be sent to;

Our guidance as a responsible provider, in setting a reasonable expectation to Health and Social Care professionals making scheduled requests, is that we need to ensure we;

  • Take reasonable steps to schedule 132 annual requests for assessments and reviews in a manner that is balanced as evenly as possible to the home.
  • Act in a way that is reasonable to the family member to ensure they do not cancel any existing booked visits and activities.
  • Act in a way that is reasonable to our clinical leadership team in the home to have sufficient preparation and visiting time available among their existing duties.
  • Have resources such as Care Record Computers available to the visiting professional in a reasonable way that does not impact the running of the home or upkeep of accurate medical records for our family members.

We would expect a professional to have received a response to their request with a selection of available dates/times within 2 working days and the dates and times will be made available at around 10-14 days from the request. In addition, the professional should expect that any required documents will be sent by email as a PDF attachment 48 hours prior to the assessment or review.

 

When things need to move a little faster.

 

We understand as responsible provider that there will be times where there is a higher degree of urgency for assessments and reviews to be completed, this may be because (but not limited to);

  • A significant change in support needs of the Family Member.
  • A short-term funding agreement coming to an end.
  • A response to a concern/complaint to the commissioning agency.
  • Connection to a legal process such as a Deprivation of Liberty application.
  • The operational need of the commissioning agency.

In these circumstances we understand it is reasonable to expect the ability to complete the assessment or review in the timeliest manner possible. We will always endeavour to facilitate these requests at the earliest opportunity possible.

Our ability to support such requests is only maintained by ensuring all scheduled requested are booked in a reasonable and proportionate manner. These more pressing requests still need to be made by emailing the assessments and contracts team with the title “unscheduled assessment/review request” to be sent to;

We would expect the worker to receive a response with a list of dates/times available for the following days within one working day.
